Saudi Arabia’s Ministry of Hajj and Umrah has made medical fitness certificates mandatory for Hajj 2027 (1448H) pilgrims.
The Health Clearance Certificate must be processed through the Nusuk Masar platform before a Hajj visa can be issued.
Pilgrims with serious health conditions, including organ failure and advanced pregnancy, will not be cleared to perform Hajj.

Saudi Arabia’s Ministry of Hajj and Umrah (MOHU) has made medical health assessments and clearance certificates mandatory for pilgrims performing Hajj 2027. The rule exists so pilgrims can safely handle the physical demands of the rites in heat and large crowds, and to protect public health across the pilgrimage.

Hajj authorities in sending countries are putting the requirement into effect through local medical screening programs.

What Pilgrims Need to Submit

Pilgrims must complete a medical fitness examination, which in some countries takes place in two stages. This examination must certify that the pilgrim is free from disqualifying health conditions.

The results feed into a Health Clearance Certificate, processed through the official Nusuk Masar platform. This certificate is a prerequisite for Hajj visa issuance, and Saudi authorities may check it at entry points.

Vaccinations, particularly the quadrivalent meningococcal ACWY vaccine and others as specified, remain a separate requirement and are not replaced by the medical clearance process.

Conditions That Disqualify Pilgrims

MOHU guidance lists health conditions that disqualify pilgrims from Hajj 2027, including:

Major organ failure, such as dialysis-dependent kidney failure or heart failure with symptoms on minimal exertion
Chronic lung disease that requires oxygen
Severe neurological or psychiatric disorders affecting cognition, or with significant motor disability
Advanced age combined with dementia
High-risk pregnancy or pregnancy in the final trimester
Active cancer under chemotherapy or intensive immunosuppressive treatment
Active infectious diseases that pose a public health risk, such as open pulmonary tuberculosis

Pilgrims must generally be able to perform the rites independently. Walking sticks for light support may be allowed, but full dependence on a wheelchair or scooter is usually not permitted.

How the Process Works

Rules can differ by country, with specific details and deadlines set by each national Hajj committee. Pilgrims should follow instructions from their national Hajj committee or the official Nusuk Masar process, rather than unofficial reports.
